This form serves to pause the contractual agreement between Rob Nickels Tennis Inc. and the individual player stated below. I wish to pause my recurring payment for the player named below and continue with this form. I understand that per the agreement that was entered upon during the submission of my registration, I am hereby providing my 15 days’ notice to cancel the existing recurring payment agreement and resume the contract on the date I will specify below. If I have not provided Rob Nickels Tennis Inc. with at least 15 days’ notice prior to the the next recurring bill payment, I understand that there will be one more payment processed for the succeeding month. If you are providing Rob Nickels Tennis Inc. with less than 15 days notice of cancellation, we welcome you to continue to attend our classes during this last paid month.
