RECURRING MONTHLY PAYMENTS*
- I understand that Rob Nickels Tennis Inc. and The City of Parkland at Parkland Tennis Center uses an Automatic Recurring Billing process and that I am currently enrolled in automatic recurring monthly payments. I understand that if my request for changes in my child’s junior schedule is approved (as opted for in this online form) and if these changes results in an increase in my current monthly tuition fees, I authorize The City of Parkland. to change my current existing monthly subscription recurring payment amount for the next scheduled recurring payment date. I understand that Rob Nickels Tennis Inc. will process a one-time charge using the credit or debit card that that is on file and used for my automatic recurring payments for any increases in my total recurring monthly amount. This one-time charge amount will be prorated based on a 30-day monthly cycle and based on the current advertised monthly fees for the chosen tennis group. I understand that if my request for changes in my child’s junior schedule is approved (as opted for in this online form) and if these changes results in a decrease in my current monthly tuition fees, I authorize Rob Nickels Tennis Inc. to change my current existing monthly subscription amount on the next scheduled recurring payment date. I understand that there will be no refunds or prorating of monthly fees if I have decreased my monthly recurring payment amount. No exceptions. PLEASE CHECK THE BOX.
AUTHORIZED USER OF CREDIT CARD*
- I certify that I am an authorized user of the credit card used in the initial junior registration form of the above name child and that I will not dispute the scheduled payments with my credit card company provided the transactions correspond to the terms indicated in this and the initial junior registration form for the above named child. There will be no refunds and no prorating of the monthly fees. No exceptions. PLEASE CHECK THE BOX.
MAKE-UP CLASSES*
- Make-up days will only be granted only if the class is cancelled by Rob Nickels Tennis Inc. or The City of Parkland. By checking the box, I understand that make-up classes will not be granted if your child is absent from a class that was not cancelled. Make-Up classes are to be attended on a different day when the same group level is offered. I understand that cancelled classes cannot be switched for private or semi-private classes. No pro-rating of future or past payments will be made in lieu of make-up classes. PLEASE CHECK THE BOX.
PHOTOS/VIDEO*
- Photo/video consent – sometimes we may take photos/videos in the tennis sessions. These may be used on promotional materials such as brochures, posters and on official social media or on our web site. At no other times will the photos or videos be made public. Photos may also be made available for you to download. PLEASE CHECK THE BOX.
LIABILITY:*
- TERMS AND POLICIES. Waiver for minors. Read carefully before checking the box. As an adult or the parent and legal guardian of the above named minor, I give permission for myself or my child to participate in this tennis program. I hereby agree to indemnify and hold harmless, Rob Nickels Tennis Inc, The City of Parkland, and its agents from and against any and all claims, demands or causes of actions of whatsoever kind, and for any resulting injuries, losses, costs, damages, liabilities, expenses and attorney's fees arising out of his/her participation in this activity.
